McCoole was able to offend over a period of three years after Families SA failed to respond to ‘red flags’ that indicated he was interested in preying on the children in his care.

McCoole was not correctly referred to police in the month before his arrest, even after a young girl in his care complained her ‘bottom hurt’.

His case sparked a royal commission into South Australia’s child care system.
