A nurse has been charged in the death of President Donald Trump’s former national security adviser’s father at a Philadelphia senior care facility.

Christann Shyvin Gainey, 30, was charged on Thursday with involuntary manslaughter, neglect and records tampering in the death of H.R. McMaster Sr.

McMaster died on April 13, about eight hours after falling and hitting his head at the Cathedral Village retirement community.

The Pennsylvania attorney general’s office says surveillance video showed that Gainey failed to conduct a series of neurological evaluations of McMaster as required.

Prosecutors say Gainey falsified documents to make it seem she had.

Gainey is a contract employee at the facility and a licensed practical nurse.

McMaster, an 84-year-old retired US Army officer, was admitted to the facility on April 9 after suffering a stroke.

His medical file noted that he was at a high risk of falls.

He fell and hit his head at the facility on April 13.

The male nurse assistant who found him on the floor helped McMaster into a wheelchair.

He told police he had informed Gainey that McMaster had trouble communicating.

McMaster had open wounds on his face and shoulder.

The nurse assistant said he never saw anyone check McMaster’s vitals throughout the night.

He said he went and got Gainey in the early hours of the morning because he wasn’t sure if the patient was breathing.

When the nursing assistant director arrived about an hour later, she was told of McMaster’s death.

Philadelphia Police executed a search warrant at the facility soon after and revealed they were treating his death as ‘suspicious’.
