Queensland Police have launched an urgent amber alert for a four-week-old baby boy who may be at significant risk.

The boy, from Fortitude Valley, in Brisbane, was last seen on Sunday.

A 37-year-old woman known to the baby was last spoken to at midday on Thursday. 

She is believed to be travelling in the Brisbane City area on foot.

She is described as Pacific Islander, approximately 168cm tall, proportionate build and with brown hair and brown eyes.

Anyone with information about the missing baby can call 131 564, or 000.
