‘Human error’ to blame after handcuffed prisoner fell out of moving police van into heavy traffic

Bungling cops who forgot to lock police van door before handcuffed prisoner escaped from the moving vehicle are reprimanded as probe finds ‘human error’ was to blame

West Australian police officers have been reprimanded after a handcuffed prisoner was filmed escaping from the back of a police van in West Perth.

Bizarre footage was posted on the Dash Cam Car Owners Australia Facebook page showing a man falling from the van’s back door while it was turning right at the intersection of Charles and Newcastle streets in November.

The man had been on the run for four days prior to his arrest in connection to an alleged bottle shop armed robbery. 

The man was captured on dash cam footage falling out of a police van onto a busy road in Perth

The 31-year-old was arrested, handcuffed and placed in the police wagon, before he was captured falling out of the car by a nearby motorist. 

He landed on the busy road where he continued to roll while still in handcuffs.

Police said officers heard a ‘thud’ and chased the man for about 150 metres on foot before he was recaptured. 

The passing vehicle which captured the footage drove away after the man fell out, missing officers chasing the prisoner up the street.

He was not injured during the incident.

West Australian police told Perth Now the man fell out of the van due to ‘human error’ and that the officers involved in the incident have been reprimanded. 

In November, WA police minister Michelle Roberts described the footage as ‘very disturbing.’
